When you call in, always state clearly, “This is an emergency.”. For poisonings, call the poison control center at 1-800-222-1222.
We strive to complete forms as soon as possible, but due to the volume of forms we receive and our focus on immediate health needs of our patients, we require three (3) business days to complete them. Most forms have sections to be completed by the guardian and/or patient, and this information MUST BE COMPLETED (including exact names of medications and the form, ie. pill, liquid, etc) before we can accept the form, and the patient must be up-to-date on recommended check-ups. The chart is reviewed by one of the medical staff, and often by your physician, before a form can be completed. If your child is seen by a specialist, please have the specialist sign the forms related to the condition or medication.
Medications that are more closely regulated by the FDA have different requirements for prescribers. We are required by law to see your child regularly to monitor medication use and usefulness.
UH Rainbow Green Road Pediatrics welcomes patients from many health plans. This list is accurate to the best of our knowledge. However, when choosing a new medical insurance plan, it is your responsibility to verify with them that we are in-network providers.
As a parent or legal guardian you have the ability to access your child's online health information. Visit the Parent/Guardian Access section under MyUHCare to learn more.
