How do I enroll in the Golden Valley Health Center program?

On a desktop or laptop select the “Enroll Now” button. If you are on a tablet or mobile device select the “Register Now” button. Enter your 8-digit token, your date of birth [mm/dd/yyyy], and your email address. If you did not provide Golden Valley Health Centers with an e-mail address, no worries!

Through the myGVMH Health Portal, you will have access to information related to services you received on or after December 1, 2018. However, you may request information from Health Information Management at any time, regardless of the date your received service. Your information will be available within 36 hours of service.
