MyFloyd patient portal is a secure website that allows read-only access to your personal health information and interact with Floyd family medicine providers. All Floyd patients are encouraged to join and use the patient portal for ready access to your personal health information.
Because we release results to you immediately, you may see results before your healthcare provider has seen them. Some results may be difficult to interpret please allow your provider time to review the result and follow up with you. If you have an immediate concern, please contact your provider.
Floyd uses different electronic medical records to provide patient care. Depending on where you receive your health care, you may need access to more than one portal. To obtain a copy of your medical record, you must contact the Health Information Management (Medical Records) department at Baptist Health Floyd at 812.948.7422. According to HIPAA rules and regulations, you are entitled to a copy of your medical record. However, there are specific laws we must follow to release this information.
Your signature is required on a release of information authorization form.#N#A copy of your photo identification is needed.#N#Appropriate paperwork must be provided for release of a family member's record.#N#We are allowed to charge for a copy of the medical record; certain variables apply.#N#We are not permitted to fax copies of medical records.
You may call 812.948.7422 to request a copy or ask questions.#N#For a copy of your X-ray film, please contact the Radiology department at 812.949.5904.#N#Please allow three to five business days to process a request.#N#Office hours are Monday thru Friday, 8:30 am - 5 pm.

Baptist Health Floyd's Patient Portal gives you convenient, online access to portions of your electronic medical record at any time. All of the information is stored securely, password-protected and delivered via an encrypted connection.
If you would like to sign up and don’t have an After Visit summary from your physician or a Baptist Health facility, visit MyChart to get an activation code. Once you have your activation code, you can setup your username and password on the MyChart website .
To login to MyChart, visit mychart.baptisthealth.com. Once on the page, enter your username and password that you setup when your created your account and click Sign In.
After logging into your MyChart account, hover over the Messaging tab and select “Ask a Question”
MyChart is a free patient portal that combines your Baptist Health medical records into one location.
If you would like to sign up and don’t have an After Visit summary from your physician or a Baptist Health facility or an activation code, you can still sign up for MyChart. If you've been a Baptist Health patient, your MyChart account will connect to your medical record.
