What is FollowMyHealth patient portal?

At First State Orthopaedics, we provide our valued patients with a secure, confidential, and easy-to-use service known as Patient Portal. We’re eager to improve your healthcare experience and provide the best care possible with this simple tool. The Patient Portal allows you to easily connect with your doctor online.

Apple Health and Medlio App

We are excited to announce a new option that allows patients to view their health information. In addition to the FollowMyHealth® App, you can now use the Apple Health and Medlio App to view and combine your health records. Apple Health and Medlio provide secure access and control over your healthcare data.

OVERVIEW

FollowMyHealth® Patient Portal is State of Franklin Healthcare Associate’s secure, online patient portal designed to help you access and manage your health information when it is most convenient for you.

ENROLLING

Sign-up is simple and takes place during the patients visit with their doctor or provider. As a safety precaution, patients must sign up in person to confirm their identity. Just ask any staff member about getting connected right away.

What is my username? My password?

You determine your own username and password when you create your FollowMyHealth® account online.

ACCESS

How can I access my child’s or other family member’s health information? If you are interested in FollowMyHealth® Patient Portal access for your child (ren) or an adult for whom you are legally responsible and that individual is a patient of a participating provider, you will need to sign up in person by completing a Patient-Designated Personal Representative Access Request form.

CONFIDENTIALITY AND PRIVACY

Will my information in the portal be secure? Yes, your personal and medical information in the FollowMyHealth® Patient Portal will be kept confidential. FollowMyHealth® uses a secure, encrypted connection that meets the highest industry standards. All health data is stored on a secure server.
