Melynda joined the Skyline family in June 2017. She graduated from the University of Colorado with a bachelor's degree in biology. She then received a Master's degree in Physician Assistant Studies from Midwestern University, Glendale, AZ in 2011.
Jared has been part of the Skyline family since 2013. He graduated from Arizona State University with a bachelor's degree in Biology and Business. He then received a Master's degree in Physician Assistant Studies from A.T. Still University, Mesa, AZ in 2012. Jared is also a Captain in the United States Air Force, Air National Guard working in the medical group. Born in Sacramento, CA, Jared grew up in Mesa, AZ. He is married with 3 handsome boys and 2 beautiful girls. His hobbies include fishing, camping, and drawing.

All new patients to Skyline Medicine will receive an annual exam during their first visit so our providers can review all medical conditions, medications, medication interactions, medical history, allergies to medications, past medical records, family history, recent hospitalizations, lab work, immunization history, living situation, and all other relevant medical information that will ensure a thorough and accurate first visit for all of our patients. During the first visit it is critical that the information provided in the new patient paper work is as accurate as possible because medical decision-making for the first and future visits depends on it. For established patients, if you are here for a scheduled office visit and it has been more than 1 year since you’ve had an annual exam with Skyline Medicine, we will perform the annual exam during this visit.
