Consistent with our philosophy it is our desire to bring the Creighton Model Fertilitycare™/NaProTECHNOLOGY to as many women and their families as possible. Because there are so few Creighton Model Fertilitycare™/NaProTECHNOLOGY providers, patients are often forced to travel long distances to get the help they need.

Fee Schedule

This remote management is considered a “non- covered service” and is not billed to your insurance company–you are solely responsible for the incurred fee which will be charged to your credit card.

How to use patient portal?

PATIENT PORTAL IS USED FOR THE FOLLOWING TASKS: 1 Gather Medical Records – Proper completion of portal ensures medical records will be transferred and received in time for physician review in preparation for the initial consultation. 2 Check Insurance Benefits – Information input in portal allows our billing team to check benefits and prepare to answer questions regarding coverage and costs of treatment. 3 Communicate between Patients & Physicians/Staff – Portal is used to send information regarding treatment and billing. Patients can use this tool to ask questions and make requests. 4 View & Pay Bills – Patients can view statements at their convenience and pay bills directly through portal. We use a paperless billing environment to do our share in protecting the environment.

Where did Melanie join the fertility and midwifery care center?

Melanie joined the Fertility and Midwifery Care Center after several years with the Parkview Physicians Group, OB/GYN and works mainly with the Nurse-midwives. After graduating from Wright State University and getting married, she and her husband moved to Columbus, Ohio where she worked with a Cardiology group at Grant Hospital. Her husband’s military career took them to Fort Sill, Oklahoma where they began their family with the birth of twins.

What is Genevieve's job at FMCC?

During and after college, Genevieve freelanced in social media and business development work for several small businesses. She had a brief experience in the corporate world of Target, but quickly missed the environment of a small business. She joined the FMCC management team in October of 2020, and is overjoyed with the opportunity to work in the family business. Her role focuses on communication, marketing, and business development.

Who is Jenna from FMCC?

Jenna graduated from Ivy Tech Fort Wayne in 2017, earning her associates degree in Applied Science in Medical Assisting. She had a desire and a passion to work in women’s health, and was excited to join FMCC soon after graduation. She married her husband Andrew in April 2020. They were overjoyed to welcome their son Asher in May 2021, who was born at Holy Family Birth Center. In their free time they enjoy spending time with family, being outdoors, and relaxing at the lake! Jenna is grateful to be able to work alongside a wonderful team that values life, and goes above and beyond to give genuine care to their patients.

Who is Cortney from Holy Family Birth Center?

Cortney is one of our FMCC medical assistants and a Birth Tech at Holy Family Birth Center. She studied medical assisting in Falls Creek Virginia and worked in Family Medicine and Electrophysiology. She began her journey with FMCC first as a patient, and had all 3 of her children with us. During her pregnancies, she fell in love with the practice and our values. After enjoying her "job" as a stay-at-home mom for 6 years, she followed her passion for birth and pregnancy to find a career in our office. Courtney is currently working towards becoming a lactation counselor and a Holy Family Birth Center Nurse. Her hobbies include drinking coffee, reading, and going on adventures with her children.

Who is Lori at Parkview?

Lori leads our front office and will always greet you with her warm smile. Lori was one of the first employees who made the decision follow us in this adventure that is the Fertility and Midwifery Care Center, joining us after working several years for Parkview Physicians Group.

Is Audra a part of FMCC?
