How do I set up my FCI account?

Setting up your account is easy and only takes a few minutes. Step 1: From any computer, tablet or smartphone, go to FCI.MyHealthPatientPortal.com and click “Register” on the login page. Step 2: Enter your date of birth and your unique patient number. Current patients get their unique patient number from their clinic.

To mail your payment to us, simply send us the top portion of your billing statement along with a personal check or your credit card information authorizing payment.

To obtain a copy of your medical records, please return a completed copy of the medical records authorization form. If you have not yet completed an authorization form to have medical records sent to you or another healthcare provider, see the link to download the authorization form. You will need to print, complete, and return the authorization to us by mail, e-mail or fax. The fax number and address for return ing the authorization can be found at the bottom of the form.

Do fertility centers of Illinois require patient records?

Fertility Centers of Illinois maintains patient records in accordance with regulatory standards. These standards do not require that all patient records be permanently maintained. Depending on the date and type of service, patient records and films may no longer be available for access and duplication. Share.
