Do you have a patient portal?

The MyChart patient portal is here. Our former patient portal in still available, so you can get to your health information and pay your bills. What is our patient portal?

To take advantage of virtual urgent care visits, patients must be registered for their own MyChart account, be age 6 or older, and be located in Wisconsin. Does ProHealth Care have a patient portal?

Many but not all of our ProHEALTH Care locations use our patient portal to communicate with patients. Check with your doctor’s office to see if the portal is available to you.

How to sign up for MyChart?

Sign up and instantly turn on your account by selecting "Sign Up Now," then "Sign Up Online." Follow the process to confirm your identity. After you confirm your identity, you'll be able to get to your MyChart account.

How to schedule a checkup with a doctor?

To schedule a checkup with your primary care doctor or a follow-up appointment with your specialist, go to the “Schedule an Appointment” section in MyChart and click on which doctor you want to see. You'll then be able to pick from a list of available times.

How to schedule a visit with a new primary care doctor?

To schedule a visit with a new primary care doctor , go to the “Schedule an Appointment” section in MyChart and click “Find a New Provider.” You will then be able to find available appointments based on clinics or providers (doctors).

How old do you have to be to access your health record?

Ages 18 and older: Once a child turns 18, parents no longer have access to their health record in MyChart and have to be granted access by the adult child.

How to find urgent care?

To find the nearest urgent care, go to the "Find Urgent Care" section in MyChart. Once you have selected "Find Urgent Care," you'll be able to see a map of the closest urgent cares, as well as each location's current estimated wait time .

What happens after a doctor joins a video call?

Once your doctor joins the video call, your visit will begin . After the visit, you'll get your After Visit Summary, which will outline the visit and contain any patient information and/or educational materials your doctor adds.

Does MyChart work with FHIR?

If you have an app that uses FHIR to work with your doctor’s electronic health record system, MyChart supports it.

Can you get a symtom at home without care?

Mild symptoms can often be addressed at home without care. If you develop trouble breathing or other moderate symptoms, call your primary care physician, visit any ProHealth Urgent Care location or schedule a virtual urgent care visit.

What is the ProHealth Patient Portal?

The Patient Portal allows patients who are registered users to communicate with their health care providers and to access certain health information.

What to do if you lose your ProHealth password?

You will contact ProHEALTH if you lose your password or if you believe that there has been an unauthorized access to your account on the Patient Portal.

Does ProHealth protect against intrusion?

Although ProHEALTH has implemented security in the Patient Portal to protect the confidentiality of your information, ProHEALTH cannot protect against each and every potential intrusion. Your expressly assume the sole risk of any unauthorized disclosure or intentional intrusion or of any delay, failure, interruption or corruption of data or other information transmitted in connection with the use of the Patient Portal.

Can you share your patient portal ID?

Your Portal ID and Password must be used in order to access your account on the Patient Portal. You agree that you will not share your Portal ID and Password with anyone unless specifically authorized by ProHEALTH to allow a family member or legal representative to have access to your Patient Portal account.

Can you use the Patient Portal for urgent matters?

You understand that the Patient Portal cannot be used for urgent or emergency matters. You may contact ProHEALTH by phone or in person for urgent matters and you should contact 9-1-1 for emergencies.

Does ProHealth have a liability?

You understand that ProHEALTH does not assume any liability for the materials, information and opinions provided on, or available through, the Patient Portal. You agree that reliance on the content of the Patient Portal is solely at your own risk. You understand that ProHEALTH disclaims any liability for injury or damages resulting from the use of the Patient Portal.
