If Your Child Is 13 or Older

Patients age 13 or older must request their own accounts and grant permission—also called proxy access—for parents, caregivers, grandparents or other legal decision-makers to have access to their accounts. Gillette staff can help you with that process. To create proxy access, you must complete a MyGillette proxy access request form (PDF).

Appointments may be requested by calling during business hours of 9 am – 12 noon and 1 pm – 5pm. Same day appointments are accomodated when possible, although there is no guarantee.
