Any patient family at those locations can enroll in MyChart by going to the practice or clinic and request a proxy account. Parents or guardians can request proxy access to their children's accounts. Patients 18 years and over may go online and Request Activation Code by completing the MyChart Access Request form.
First call Medical Records at Texas Children's Hospital at 832-824-1600. They will get you the forms you need to allow your medical record to be sent to your new health care provider. Medical Records will either mail or fax the forms to you. Note, you might have to wait up to two weeks to get the forms in the mail.

Parents will no longer have access to their child’s information once the patient turns 18. The patient cannot grant access to a parent to restore access. My child with special needs is over the age of 18.
Can I set up a MyChildren’s account? Yes, as long as you have guardianship papers on file with Boston Children’s , you can access your child’s information with your MyChildren’s account. If you have the guardianship papers, you can email the PDF documents to mychildrenssupport@childrens.harvard.edu.
