What is an ezderm app?

EZDERM is a free, stand-alone companion app that can only be used with a web-based service. EZDERM is a truly mobile, cloud-based Electronic Health Record (EHR) designed specifically for dermatologists and their staff.

What can you do with a biopsy form?

Users can quickly schedule patients, record allergies and medications, and generate biopsy requisit ion forms. In addition, users have the ability to take and draw on photographs, use robust voice recognition, and utilize a vast array of features designed to make the user’s office more efficient and productive.

Is ezderm the best platform?

I have used multiple platforms, including MODMED, and I have to say.... EZDERM Is truly the best I have seen and used. From the training alone it was light years above anyone else. But the platform.... from always updating to what you need, to the user friendly body map. I wish I could give more stars. EZDERM is truly the way to go.

What to bring to a medical appointment?

This should include all prescriptions, including pills, injections, creams and ointments. Furthermore, please bring a list of items that you are using for your daily skin regimen.

Do webcams have better resolution?

Get the best internet connection you possibly can: camera phones and webcams tend to have better resolution than built-in computer cameras—IF you are on wi-fi. Find a reliable wi-fi connection. Find a location with good lighting.

Is a dermatologist's visit confidential?

Understand that your visit is confidential and you should both ask any and all questions you may have as well as share any information that may be pertinent to your skin condition. Be prepared to show the doctor the areas of your concern. A gown will be provided to you so that you may undress to your comfort level.

Do doctors need you to come in for evaluation?

Be advised that there are some conditions where the doctor may still need you to come into the office for better evaluation, testing, or treatments.

Do you have to be in a private room when you are at work?

If you are at work, you may want to find a private room. The doctor will be in a private area on our end.

Preparing for Your Appointment

We ask that you please check in online through our EZDerm patient portal powered by Sadio prior to your appointment. When you make an appointment, you should have been sent an email linking you to your secure patient portal on EZDerm.

Cancellation Policy

We ask that you please give notification of cancellation 24 hours prior to your scheduled appointment. Failure to do so may result in a $50 No Show Fee that must be paid prior to the scheduling of your next appointment. Please be aware that Monday appointments must be cancelled by noon on the previous Friday.

Being on time is very important to us.

Payment Options

We accept most insurances and submit insurance claims. Our office financial policy is that all co-pays, co-insurances, and deductibles are collected at the time of service. All cosmetic and laser procedure fees are collected in full at the time of service. We accept MasterCard, Visa, Discover, American Express, and CareCredit.

Gift Certificates

Gift Certificates must be present at the time of service. Gift Certificates are non refundable and non transferable. Gift Certificates cannot be used for medical treatments. Revival Dermatology is not responsible for lost or stolen Gift Certificates.

Return Policy

Skin Care products (unopened) may be returned within 14 days for a full refund. Skin Care products (unopened) may be returned between 15-30 days for office credit ONLY. NO returns will be accepted after 30 days.

What is the email address for Ezderm?

New patients- you will receive an email from noreply@ezderm.com with easy instructions on how to set up your portal access.

How long before a patient visits, should you fill out intake form?

Ideally, 24-48 hours before your anticipated visit, we will confirm that your insurance is active. You should fill out your intake information through Sadio, our secure, patient healthcare portal so that your visit can occur promptly at your scheduled time.

What is credit care on file?

Opting for credit care on file via Square enables touch-less payment of any balances or extra costs incurred. To learn more about touch-less pay, click below.

Why do new consent forms populate?

New consent forms may populate due to change in office and statewide policies.
