Visit the Medical Records Department at East Alabama Medical Center or EAMC-Lanier with your photo ID and a completed patient portal authorization form. A valid email address is required for accessing your patient portal.
Once you have created your account, log in to your MyOSH Patient Portal with your email address and password that you created . Patients who have signed up for the MyOSH patient portal can access details of their medical records by using the HealtheLife app.
Click the blue link in the email. You will then enter your birthday and the last four digits of your social security number to gain access to your My EAH account. Visit the Medical Records Department at East Alabama Medical Center or EAMC-Lanier with your photo ID and a completed patient portal authorization form.
