How do I get my medical records from Colorado Mountain Medical?

Please complete a release form and submit via email (located at bottom of form), fax or drop off to one of our Colorado Mountain Medical locations. Medical Records will be available for pick up. If immunization records are needed, please access your Colorado Immunization Information System (CIIS) portal here.
