For medical records requests, please contact our Medical Records Department at Memorial Hospital – our team can be reached at (937) 578-2365 from 8:00am to 4:30pm, Monday through Friday. Alternatively, medical records requests can be submitted through MyChart.

Do providers have access to health information?

Our providers will still have access to your health information but we wanted you to be aware so that you could maintain your personal health records.
