How long does it take to get Supplemental Critical Illness Benefit?

Supplemental Critical Illness benefit available for full-time or part-time (.5 FTE or above) employees. May only be enrolled within 30 days of the new hire period, within 30 days of a qualified life event or at annual open enrollment. The Critical Illness benefit is a lump-sum payout depending upon the diagnosis of certain critical illnesses, which meet the coverage criterion.

What is SIP 403b?

The CoxHealth Savings Incentive Program (SIP) 403 (b) is available to all employees working a minimum of 1,000 hours per year upon hire and is designed to help you reach your retirement goals. The plan offers tax advantages as well as one of the most generous matching contributions among area employers. This is an excellent way to build up a solid retirement fund through your working years.

What is Supplemental Accident Insurance?

Supplemental Accident Insurance is available for full-time or part-time (.5 FTE or above) employees. This benefit helps pay for out-of-pocket expenses that result from off-the-job injuries and accidents. Can be enrolled any time but takes effect the first of the month following the month after enrollment.

How often do you have to pay for eye exams?

A co-pay for an eye examination every 12 months, a co-pay for lenses every 12 months and frames every 24 months. The effective date is the first of the month following qualified enrollment or first day of plan year following open enrollment.

What is ETO in the US?

ETO is used any time an employee is off work during his/her scheduled work hours unless the employee qualifies for Accumulated Disability Benefit, Funeral Leave or Jury Duty. ETO will be awarded each pay period based on the prior pay period hours paid, excluding weekend option bonus hours. An employee may accrue up to two times his/her potential annual ETO amount.

How long is short term disability?

Short-Term Disability is available to full-time or part-time (.5 FTE or above) employees after 30 days of continuous employment. Plan has guaranteed issue for newly eligible participants. Applications after guaranteed issue period are subject to underwriting and plan provision. Premiums vary depending on age and amount of coverage. The benefit includes 60% of base salary up to a maximum of $1400 a week. Subject to medical necessity, pre-existing limitation, and plan provisions. *

How many hours can you accrue for a full time employee?

Eligible employees can accrue .0346 hours for each hour worked per prior pay period up to 80 worked hours, which can accumulate to 72 ADB hours (9 days) per year, with a maximum of 480 ADB hours per employee or the equivalent of 12 weeks of paid leave.
