What is Apple Health Core Connections?

The Apple Health Core Connections program is a new managed care program that will provide coordinated health care services for children and youth in foster care or adoption support, and young adult alumni of the foster care program. Coordinated Care staff will help enrollees get access to needed health care services, including mental health and substance use disorder services.
