Community Health Center of the North Country's Patient Portal offers patients easy and private access to their own medical information on their personal computer or mobile device, so patients can view their own health record whenever and wherever they have access to the internet.

Community Hospital North's pastoral relations department is committed to providing support services to patients, families and hospital staff, with respect to their own needs and traditions. Professionally trained chaplains are available on an individual basis to assist with pastoral/spiritual care and counseling needs.

Cell phones are allowed in all areas except for patient treatment areas because their signal could interfere with medical equipment in use.

Download both the Request for Access Form and the Release of Information Form, fill them out and send back to the Health Information Department. Completed forms can be faxed to 616-847-5599 or emailed to HIMPatientPortal@noch.org, or mailed to North Ottawa Community Hospital HIM Department 1309 Sheldon Road Grand Haven, MI 49417.
