Is Patient Portal compatible with mobile?

The Patient Portal website is compatible for use on various devices. In addition to a desktop site, a mobile site with a layout optimized for a smaller screen size is also available. The menus and screens are programmed to be easy to use on a smartphone and an icon can be placed on the Home screen for quick access.

How long does it take for a pharmacy to process a refill request?

Please remember to allow 48-72 hours for the practice to process your refill request. You can also notify the practice of medications you are no longer taking or of any new medications. You can use the portal to notify the practice if you change your pharmacy.

Where is Kelsey Cone?

Kelsey Cone, DMD completed her undergraduate studies at Norwich University in Northfield, Vermont. There, she was a student-athlete and the assistant captain of the National Championship ice hockey team. She then attended University of New England College of Dental Medicine in Maine as a member of the inaugural class. Dr. Cone completed her General Practice Residency at Community Regional Medical Centers of Fresno, California. Her exceptional training and exposure to multiple local hospitals serving diverse populations has afforded her invaluable experience and knowledge in all aspects of general dentistry. Dr. Cone has a strong passion for helping others. With her meticulous treatment planning and execution, she constantly strives to make a positive and lasting impact on her patients. She is committed to always doing what’s best for her patients. Dr. Cone enjoys the outdoors with her dog Wilson and plays golf in the off-season. Her latest interest is following the Portland Trailblazers. Go Blazers!

Where did Shelly work?

Shelly spent eight months working in Haiti prior to obtaining her Doctor of Nursing Practice at the University of Portland, Oregon in 2017, and completed a Nurse Practitioner Residency program through Yakima Valley Farm Workers in 2018.

What is Lauren Carpenter's background?

Lauren Carpenter, LICSW uses a collaborative and strengths-based approach in her therapeutic work, incorporating aspects of cognitive behavioral therapy and mindfulness to help people reach their goals. Prior to joining CHCB, she worked in a number of mental health settings, including a women’s mental health clinic supporting pregnant and postpartum women, and an early childhood mental health clinic where she worked with families with young children. She graduate with a BA from Smith College, and an MSW from the University of Maryland, Baltimore.

How many levels are there in the parking garage?

Our parking garage has two levels with lots of spaces for our patients. Once you park in the garage you can take the elevator up to Level 1 for Dental and Patient Counseling or Psychiatry appointments or Level 2 for Medical appointments.

Medication Refills

For prescription refills, please contact your local pharmacy or call 802-864-6309 to be transferred to our medication refill line. Requests will be addressed within two business days (48 hours).

Medical Records

To obtain past medical records from CHCB, please call 802-264-8125. To send medical records, please fax with a cover letter to 802-860-4313.
