What is self enrollment in Boone Health?

The automated self-enrollment feature allows patients to enroll in Boone Health Connection with information that has been verified at a recent visit. If the system cannot match your submitted information to an account with 100% accuracy, the enrollment will fail. To protect our patients’ health information, enrollment requests that are not successfully matched are flagged for review by our Portal Team. An unsuccessful enrollment request does not preclude participation in the patient portal, only that a team member must review your information to correctly link your clinical account.
