Q: How do I change my card on file?

A: To change your current card on file you must swipe the new card on the Health iPASS kiosk located in the clinic.

Q: What happens if the card I have on file declines for any reason?

A: The card is no longer valid. You will need to swipe the card again the next time you are visit your clinic.

Q: How often will I need to swipe my card that I wish to keep on file?

A: Once for shots (until the card expires) and each time for an Office Visit.

Q: Does CAAC Staff have access to my card information that is on file?

A: No, your card information is securely stored within the Health iPASS System and is not available to anyone.

IMPORTANT PLEASE READ: If you are a new patient or require testing, please stay off antihistamines for 7 days prior to your appointment. Also, if you are an established patient but have not been seen for over 3 years, you will need to schedule a New Patient Appointment.

What is a referral for insurance?

Referral. A number of insurance plans require a referral or authorization to be seen by a specialist. As we are unable to determine the specific requirements for all insurance plans, please work with your primary care physician and insurance company to complete the referral or authorization process.

Does Colorado ENT have a MyChart account?

Please note, Colorado ENT & Allergy leases its patient portal from Centura Health and, therefore, you may have already created a MyChart account. If so, you will use your existing login rather than signing up for a new account.

How to fill out a new patient form?

You have several options when filling out your new patient forms: 1.) You can fill out the form electronically within your web browser, just make sure to complete on a desktop or laptop computer and press submit at the end. It will be delivered directly to your preferred location. 2.)

How long does it take to get an allergist to evaluate you?

The time and cost of this visit will vary based on your symptoms and the testing that your allergist recommends. Generally the evaluation takes between 2-3 hours. Because of this, it is vital to avoid taking antihistamines for 7 days prior to your visit.
