The Medical Records Release Request Form is a single use authorization form where the Patient gives permission to Clarksville Medical Group, P.A. (CMG) to proceed with the disclosure of Protected Health Information (PHI) using only one (1) of the two (2) options described below.
Signing up for the Patient Portal couldn’t be easier: all you need is a valid e-mail address and an internet connection! To enroll, use the form on this page or stop by the front desk and speak to any of our staff . They will enter your e-mail address into our system and provide you a secure ID along with instructions on how to finish the enrollment process at home or on your smartphone.

Use the Appointments tab to view, reschedule or cancel upcoming appointments. You can grant temporary or permanent access by choosing an expiration date in the “Set Expiration Date” dropdown menu or selecting the “Never Expire” check box. Don’t forget to tell the person you’re inviting the password created in the “Password” field. The password will not be sent to them via email.
