Immunizations are no longer available in Phoenix Children’s patient portal at this time. To access immunization information visit the Arizona Department of Health Services (ADHS) webpage. Please note this may not be a complete immunization record due to limitations with reporting to ADHS.

You can request a copy of your child's medical record from the Patient Portal. To do this, just log in to their portal and review the Request for Medical Record Release form for instructions on how to submit your request.
You may communicate with the rehabilitation/therapy team regarding routine issues or questions related to your child’s PT, OT and speech-language pathology services.
Access to your child's electronic health record varies by age: Under 14: Parents have full access to the child's MyChart account. Ages 14-18: In order to provide confidentiality for our patients, parents have limited access. Parents can continue to message the care team and see test results.
MyChart offers Children's Colorado patients personalized and secure online access to portions of their medical records. It allows you to securely manage and receive information about your child's health. We encourage parents and patients to sign up for MyChart either over the phone when scheduling an appointment or with a team member during a visit.
18 or older: Parents cannot create a MyChart account. The patient may have their own MyChart account. Special needs: If a child has diminished capacity and is unable to make healthcare decisions, an exception can be made that allows parents to retain full access to their child's MyChart account.
