All CHC providers are enrolled in the Patient Portal. If you are not a patient of Chesapeake Health Care, your request for access will be revoked. CHC does not release records from non-CHC providers to the Patient Portal. When can I see my test results in Patient Portal?

Log into Patient Portal and locate the Appointments button on the left when you log in and then click Schedule Now. Choose which Days of the Week, Times , and a detailed Message of your request. The practice will contact you within 48 business hours to approve and/or schedule an appointment for you.
While logged into Patient Portal, if your keyboard remains idle for 15 minutes or more , you will be automatically logged out of Patient Portal. We recommend that you log out of your Patient Portal session if you need to leave your computer for even a short period of time.
Patient Portal clinical information comes directly from the electronic medical record. Please contact your provider practice, or on your next visit, ask the practice to correct any inaccurate information. Your health information is reviewed and updated in your electronic medical record each visit. Remember that your diagnosis on arrival my not be the same upon discharge.
CHC has the right to revoke proxy access as needed.
Patient Portal demographic information can be updated by the patient. My Profile screen gives the patient opportunities to edit. That information does not update the patient’s record automatically. The practice will verify the information before updating the patient record.
