What is a patient portal?

ChARM PHR is a Personal Health Record (PHR) portal to manage health information of patients, for themselves and their families. Patients can store their current and past medications, lab results, record health vitals and track progress using the built-in reports.

What is a patient portal account?

Patient Portal Login Once the account registration completes, you will be automatically redirected to patient portal and also you will get a Welcome Email. Welcome email has a link to patient portal login page of your practice. You can bookmark this link for future reference.

Patient Selection

After signing in you will land in Patient Selection page. This page shows the list of all the family members registered with your current logged in account. Select the patient whose information to be viewed.

Practice Selection

When you select the patient, the practices which the patient had already visited, are shown in the right side. Select the practice link to proceed.

Patient Portal Registration

You can register a patient portal account by accepting the invitation email sent by your practice.

Choose your preferred username (Login ID) and password. Username and Password are case sensitive. Hence make sure that you enter them in proper case.
