Note: ChARM PHR employs strict privacy rules. No one can look at the data except you or those designated by you. After signing in you will land in Patient Selection page. This page shows the list of all the family members registered with your current logged in account. Select the patient whose information to be viewed.
On choosing the practice, you will land in “Dashboard” view of the patient portal, which will have the following sections On clicking the "Practice Details" icon, you will be shown the following page, which will show the facility details, provider details and other useful information about the practice.

Once the account registration completes, you will be automatically redirected to patient portal and also you will get a Welcome Email.
After signing in you will land in Patient Selection page. This page shows the list of all the family members registered with your current logged in account. Select the patient whose information to be viewed.
When you select the patient, the practices which the patient had already visited, are shown in the right side. Select the practice link to proceed.
You can register a patient portal account by accepting the invitation email sent by your practice.
Choose your preferred username (Login ID) and password. Username and Password are case sensitive. Hence make sure that you enter them in proper case.
Quick Menu has action links that are frequently used for practice communication. This can be invoked irrespective of the current section you are in.
Timeline is a new way of presenting your practice interactions based on the timeframe in which events occurred. It gives you a historical snapshot of all your practice interactions in one single convenient location.
