What happens when you activate your chart account?

Once your My Chart account has been activated, your LCMC Health Patient Portal account will become the primary vehicle to receive important messages from your participating provider (s) and other personal medical information. You MUST provide us with your email address when activating your LCMC Health Patient Portal account and keep your email address current using the Preferences option within LCMC Health Patient Portal to ensure that you receive notification of newly released information in a timely manner. You will continue to receive communications via the United States Postal Service or other means from LCMC.

How to enroll in LCMC?

To enroll in LCMC Health Patient Portal, you will use your access code. The access code you receive will give you the opportunity to create a personal login name (called “LCMC Health Patient Portal Username”) and a password. Once this access code has been used or after 60 days of assignment, it will no longer be valid.

You may discontinue your use of the LCMC Health Patient Portal anytime by submitting an online request through customer service or calling the toll-free number 866-662-6161.

Can you send urgent medical information to LCMC?

YOU MUST NOT SEND ANY MESSAGES REQUIRING URGENT ATTENTION USING LCMC Health Patient Portal. Doing so may delay your receiving necessary medical treatment and could result in loss of life, organ or limb and could cause significant deterioration in your health. If you believe you have an urgent medical matter, you should call 911 immediately or go to the nearest emergency room. If the matter is not urgent but requires a timely response, please call your physician’s office.

Does LCMC protect against inadvertent disclosure?

While LCMC uses state-of-the-art security, no system can guard against all risks of intentional intrusion or inadvertent disclosure of information. When using the LCMC Health Patient Portal, information will be transmitted over a medium that is beyond the control of LCMC or its contractors.
