How do I make payments to Calvin University?

You will find Health services in the Hoogenboom Health and Recreation Center. Our main entrance is on the west side of the Spoelhof Fieldhouse Complex. Office hours are 8 a.m. to 5 p.m. (Mon-Fri) during the school year. To make an appointment, please call 616-526-6187. Most appointments can be made within 1-3 days, as needed.

What is Calvin LifeWork?

Where is serum stored?

The serum may be stored at Health Services. Moreover, a university physician must be present when injections are administered, so appointments are required. This screening form will identify students who are at high risk for Tuberculosis (TB). A TB test is required for students who meet certain criteria, such as international students, those who have traveled abroad, or students who have worked in a high-risk setting. Do you need a physical for varsity?

A physical examination is not required, however if you plan to participate in varsity athletics, please contact the Kinesiology department for guidelines and requirements for sports physicals. Please note that if you are a student-athlete, your athletics physical will not be accepted in lieu of the required forms listed above.

Do international students need medical insurance?

International students are required to enroll in the Student Health Insurance Plan in order to meet the university’s minimum insurance standard. Do not purchase other travel insurance or other emergency-only plans. They are not considered comparable, and do not meet Calvin University’s minimum insurance guidelines.

After you receive notficiation that you have been added to your student's account, follow these steps to create your account.
