How do I access the patient portal for Concord hospitals?

If you have received your COVID-19 vaccine outside of BU’s vaccination clinic, you can upload your vaccination documentation via the Occupational Health Connect Portal. Before you begin , you’ll want to take a photo or scan in your vaccination documentation, typically this is the CDC COVID-19 Vaccination Record Card.

COVID 19 – Symptom Monitoring Survey

Faculty and staff who will be coming to campus are required to complete the COVID 19 – Symptom Monitoring Survey. Faculty and staff who are assigned categories 1-3 will receive an email from ohcinfo@bu.edu every morning with a link to the Employee Portal where you will fill out the survey prior to coming to campus.

Compliance Status

When you submit the survey, the next screen will display a message that you are either cleared to come campus or not cleared to come to campus based on your responses.

When do parents have access to their health records?

Parents only have access to your health record if an account was created between the ages of 0-13. Parents’ access are automatically removed from the patient account when they turn 16. Once the patient turns 16, they can re-consent to allow their parents to have permanent access to their health record.

Can a 14 year old remove their parents from their account?

Yes, patients aged 14 and older can remove parents/legal guardians’ access to their patient account at any time. This can be done by visiting the Health Information Department within the hospital.
