My Care Hospital Enrollment

In order for you to enroll, you will need to speak to a hospital representative. An active email address will be required for this setup. After you've spoken to a hospital representative, you will receive an email like the one shown below:

Self Enrollment from Home

To Self Enroll in My Care, you will first need to have some personal information ready. Be prepared with your full name, date of birth, and Medical Record Number (MRN). Without the MRN, you will need to contact a hospital representative. These fields will be compared and must match the information in our hospital record.

Email Exchanges

In order to protect your personal information, Parkview Medical Center (PMC) discourages the use of regular email for patient correspondence. Email messages originated by you, our patient, are not sent in a secured, encrypted format which may result in your personal information being exposed to others.

What if I have problems signing up?

We are here to help get you enrolled. Submit an online request or call us at (865) 374-5260, Monday through Friday from 8 a.m. until 5 p.m. If you are asked to leave a message, we will call you back as soon as we can during business hours.

Medical records prior to July 2018

Hospital records from before July 2018 can be obtained by calling Covenant Health Information Management at (865) 374-5269.

Claiborne and Cumberland Medical Center medical records
