Portal passwords are encrypted and URLs are re-written so that they cannot be copied and pasted.
The information in the “My Health” Hospital Patient Portal comes from your Brooks or TLC electronic health record. This ensures that you have access to the most accurate, up-to-date information possible.
During your hospital stay or outpatient visit, the hospital will provide you with the information you will need to enroll on the Portal. Occupational therapy is designed to help patients improve their ability to perform daily activities. At Brooks Rehabilitation, our occupational therapy team assesses a patient’s performance skills and develops a treatment plan specifically catered to strengthen those skills. Treatment plans can include adaptive equipment and usage training, as well as guidance for family members and caregivers. Treatment also may include comprehensive home and job evaluation, which provide a basis for any adaptation recommendations that might be necessary.
