There are two easy ways to sign up for access to the Bristol Hospital Patient Health Portal: 1. During your registration process at the hospital by providing your photo ID and a valid personal email address. OR 2. Present your photo ID and a valid personal email address to staff in the Health Information Management Department.

Can a covered entity send phi to another person?

An individual also has a right to direct the covered entity to transmit the PHI about the individual directly to another person or entity designated by the individual. The individual’s request to direct the PHI to another person must be in writing, signed by the individual, and clearly identify the designated person and where to send the PHI. The same requirements for providing the PHI to the individual, such as the fee limitations and requirements for providing the PHI in the form and format and manner requested by the individual, apply when an individual directs that the PHI be sent to another person.

As of Oct. 1, 2020, all Ballad Health hospitals, Ballad Health Medical Associates (BHMA) and Mountain States Medical Group (MSMG) clinics use MyChart with one exception: Mountain View Regional Hospital.*

For example, if you visited the Franklin Woods Community Hospital emergency room today, the ER doctor could see what medications your Ballad Health Medical Associates family physician prescribed to you at your last annual visit.

Can you edit your health record on OWL?

Do you have to be a patient to register for MyChart?

Remember: You must be a current or past patient and have a medical record at a Ballad Health hospital or clinic to register for MyChart.

Does Wellmont have a MyChart?

If, in the past, you set up a MyWellmont account with a legacy Wellmont Health System facility, your health records have transferred over to what is now called MyChart. You will just need to update the MyWellmont app through your app store.

How to grant access to MyHealthHome?

To grant a caregiver, guardian or trusted family member / friend access to your MyHealthHome account, the requestor will first need to create their own MyHealthHome account. Once they have created their own account, they can request access to your account. You then will receive notice of the request along with an auto-generated PIN code to share with the requesting individual. Once the requesting individual has entered the PIN code, the accounts will be linked and the requestor will be granted access to view your health information.
