What is the patient portal (mychc)?

Our patient portal, myCHC, gives you 24/7 access to your medical record, appointment history, test results, and more. myCHC is a secure way to talk to your provider, request referrals, see upcoming appointments, and see a summary of your visit. You can also download the healow app to use directly on your phone.

Bluegrass Community Health Center is a primary care facility. We offer MAT services as part of comprehensive primary care services. We are a patient centered medical home. We have in-house behavioral health, peer support and treatment groups available.

Information

Inpatient treatment is the preferred option for those looking to get away from their current temptations and focus completely on sobriety with no distractions. Inpatient is very similar to short-term residential treatment, but usually takes place in a clinical or hospital setting and care is provided by more clinicians and healthcare professionals.

Does Bluegrass Dermatology accept credit cards?

Bluegrass Dermatology can accept credit card, cash, or check payment for the co-pay amount, which is due at the time of your visit. A complete list of your current medications.

Do you have to have paperwork completed for a new patient?

All New Patients must have their paperwork completed PRIOR to arrival due to limited space and wait time in our lobby. You may be asked to reschedule your appointment if your paperwork is not completed.
