Once you’re on the “More Options” page, click the patient dropdown menu, and select the person’s name, which will be displayed on the “Upload/Download CCD Format Health Data” page. Use the dropdown at the bottom of that page to select either PDF or XML.

How do I sign up for MyChart?

If you would like to sign up and don’t have an After Visit summary from your physician or a Baptist Health facility or an activation code, you can still sign up for MyChart. If you've been a Baptist Health patient, your MyChart account will connect to your medical record.

You also may import information from MyChart into other applications that, with your permission, can use that data to help you manage your care, medications, nutrition and more.

A Long Term Acute Care Hospital (LTACH) serves the needs of patients with medically complex conditions, especially those suffering from multisystem complications and needing extended recovery times – a hospital stay that is often 25 days or more. A patient is usually admitted after a short-term, acute-care hospital stay.
