How do I self-enroll in Aultman OneChart?

You must be 18 years or older to use self-enrollment. Self-enrollment is for patient use only. You will need your medical record number to enroll. You can find this number on your discharge paperwork. Please contact Medical Records at 330-363-6256 if you need to obtain your medical record number. You can enroll by clicking here.

Who do I contact if I feel the information in my personal health record is incorrect?

Please contact Aultman Medical Records Monday through Friday between 8 a.m. and 4:30 p.m. at 330-363-4244.

How can I access my information once my account has been set up?

You can access your information by going to aultman.org/AultmanOneChart and entering your log in credentials.

When can I expect to receive my invitation to Aultman OneChart?

Your invitation will be sent at the time of registration. You will receive a reminder email if you have not set up your account after seven days. The invitation will expire after 90 days. Please email patientportal@aultman.com if your invitation has expired and a new invitation will be sent. This email address is monitored 8 a.m. through 4:30 p.m.

Can I access Aultman OneChart on my mobile device?

Yes, you can access the portal via your smartphone or tablet by going to www.aultman.org/AultmanOneChart.

Who do I contact if I have a question about my medications or lab results?

Please use the “Learn More” link at the top of the ‘Medication and Results’ section. If you have further questions after visiting these references, please ask your physician during your next visit.
