Associates may purchase vision insurance coverage through Vision Service Plan (VSP). VSP offers a large national network of optometrists and ophthalmologists for your care and you have a choice of using In Network or Out of Network providers.
We offer basic life insurance at no cost to full time associates. The value is one times your annual salary (maximum benefit $100,000).
Associate Coverage: from a minimum of $20,000 to a maximum of $100,000, but not more than 500% of annual earnings, without medical evidence during eligibility period. Spouse Coverage: From a minimum of $10,000 to a maximum of $25,000 without medical evidence during eligibility period, not to exceed 50% of the associate’s amount of coverage. Dependent Children: eligible for $10,000.
Voluntary Short Term Disability provides you weekly benefits payments to help replace lost income should you become disabled due to non-occupational accident or sickness. Benefits are payable on the first day for accidents and eighth day for sickness or pregnancy, for a maximum of 13 weeks.
Eligibility: Full-time associates at no cost to the associates. 90 day waiting period Benefits payable to age 65 60% of monthly salary is covered to a maximum of $5,000 per month of benefit Partial disabilities are covered after the waiting period has been satisfied

MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to: Request medical appointments.
Your test results are released to your MyChart account after your physician has reviewed them. This is generally within 1-12 days.
We aim to protect your privacy and security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more , you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.
