Right now, Aspen Valley Primary Care, Cardiology, Ophthalmology and Otolaryngology (ENT) are available through the Patient Portal. The entire hospital will soon be able to be accessed here as well.

Our Patient Portal Gives You Secure, Online Access to Your Health Information for Visits to Aspen Valley Primary Care, Cardiology, Ophthalmology and Otolaryngology (ENT)

Log in to the Patient Portal to quickly and easily access your health records. If you have not been given access to the Patient Portal, please contact your physician’s office. Access will occur upon your first visit to the following clinics: Aspen Valley Primary Care, Cardiology, Ophthalmology and Otolaryngology (ENT).

Authorization for Release of Information

Please download the form, fill out and return to the Aspen Valley Primary Care clinic prior to your upcoming appointment. The form can be taken to either Primary Care location in Aspen or Basalt, or faxed to the Primary Care clinic at 970-927-3915. If you have any questions, please contact Primary Care clinic staff at 970-279-4111.

Office Hours

If you need us before or after our regular office hours due to an emergency situation, please call 911. If you need us for a non-urgent matter that does require after hours attention, please call the office closest to you to be connected to the on-call physician.

Arriving For Your Appointment

New patients are asked to arrive 30 minutes early to complete paperwork. You can download and print the patient forms from each practice's website and complete them prior to your appointment.

Billing

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

Refill Requests

We are happy to help with refill requests during regular office hours, so please call your doctor's office to let us know what you need.

After Hours & Emergencies

If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call the closest office to you and our answering service will assist you.

Nicole Bostic, MD, FAAP

Dr. Bostic is very passionate about her patients and their medical care. She originally practiced in the South and eventually made her way to California. When she is not busy being a Pediatrician, she enjoys spending time with her family.

Merlin Sunny, DO, ABFM

Dr. Sunny was born in India but grew up in Dallas, TX. She has a big family and loves spending time with them. She enjoys action movies, music and especially singing. In her free time she loves to indulge in a good book or Netflix TV series.
