The Aspen Valley Hospital Patient Portal gives you access to your dashboard, which allows you to view current medications, your latest lab results, your medical records and any other patient specific updates. This gives YOU the control of obtaining your medical history at any time.
Right now, Aspen Valley Primary Care, Cardiology, Ophthalmology and Otolaryngology (ENT) are available through the Patient Portal. The entire hospital will soon be able to be accessed here as well.

You can download and print the patient forms from each practice's website and complete them prior to your appointment. If you're a current patient or have already filled out your forms, we ask that you arrive 15 minutes prior to your scheduled appointment time.
If you need us before or after our regular office hours due to an emergency situation, please call 911. If you need us for a non-urgent matter that does require after hours attention, please call the office closest to you to be connected to the on-call physician.

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.
We are happy to help with refill requests during regular office hours, so please call your doctor's office to let us know what you need.
If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call the closest office to you and our answering service will assist you.
