Patient Forms

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms with you to your appointment.

By completing your medical history survey online with the DigiChart Portal, you allow us to provide you with more prompt and comprehensive care.

How To Sign Up

Prior to your visit, you will be provided a username and password in order to access DigiChart. Although this tool is designed to be a convenient resource for our patients, it should not take the place of any important communications about your care. We continue to encourage you to call our office as necessary.

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – AllianceHealth Woodward will send a text and / or email to the email address and / or cell phone number you provided upon registration to the hospital. Follow the instructions in that message to create your account.

Connect to your MyHealthHome Account

If you have already signed up for MyHealthHome, click here to login to your account.

Note to previous Portal Users

Previous Portal Users will need to create an account with MyHealthHome in order to continue accessing their personal health information. Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that.
