How do I access information from the previous patient portal?

Community Medical Associates. Starting June 28, 2021, CMA patients will transition to the same Patient Portal that all Lawrence General Hospital patients currently use. To complete your enrollment, follow this link to set your patient portal user ID and password. CMA will archive each patient’s entire medical record collection for exactly one year.

Covid Vaccination Update

Alliance’s community-wide Covid-19 vaccination program comes to an end on 1/31/22. The last first dose for non-patients will be on 1/10/22. We will continue to provide 2nd and booster doses through 1/31/22. Use the scheduler here or call us at 1-707-433-5494 to schedule.
