What is Crozet family medicine?

On November 2, 2021, Crozer Health will launch a consolidated patient portal, called HealthLink, for both inpatient and outpatient care. This technology allows us to have faster and more convenient access to our patients’ records wherever they go in the Crozer Health system and provide them with more efficient and effective care.

How do Crozer health patient portals work?

UVA Family Medicine and Specialty Care Crozet provides complete and continuing healthcare from conception to old age. Hormone Therapy

We prescribe gender affirming hormones on an informed consent basis. We can connect you with affirming mental health providers in preparation for surgery, transition support, behavioral interventions, or ongoing therapy, if you are interested.

Paying for Services

Patients without health insurance or with limited financial resources can apply for aid through the billing office.

New Patients

Call our office to make an appointment. To transfer your care, ask your current provider to fax your medical records.

Your Medications

Ask for prescription refills or renewals at the time of your visit. If you need renewals between visits, you can use MyChart.

Lab Results

Most results can be viewed using MyChart. Your doctor may call you if there is an issue with your results.

Medication Refills

Medication refills can be requested between 9:30 AM and 4:00 PM each weekday. Please have the medication name, strength, supply needed (30-day or-90 day), pharmacy, and patient name readily available. Requests made after 4:00 PM may not be filled until the following business day. Please make your refill requests early in the day to avoid delay.

Fees and Insurance

UVA Family Medicine and Specialty Care Crozet operates on a fee-for-service basis. To minimize paperwork and to help keep our charges as low as possible, we ask that you pay all eligible co-payments and non covered charges at the time of your appointment.

Emergencies

If you or a family member becomes ill as a result of a stroke, heart attack, or other potentially life threatening condition, call 911 or your local rescue squad.

What is preventive care?

Preventive care, including adult health maintenance visits, well child checks, and school physicals. Gynecologic care, including pap smears. Access to your own physician when you have an urgent question or concern. When needed for urgent medical concerns, be seen in the office that day or the next business day.

Can you call your own doctor over the phone?

For medical conditions that can be safely managed over the phone, your own physician will promptly return your call or portal message, talk with you, and make treatment recommendations without you needing to take time away from work or home to come in to clinic.

Does direct primary care have insurance?

In the Direct Primary Care model, no insurance is billed for your primary care. For an affordable monthly membership fee, all of your primary care needs are covered and you have direct access to your own physician.
