Is the patient portal different from my pm pediatric care telemedicine account?

IMPORTANT: The patient portal is different from your PM Pediatric Care Telemedicine account. Your test results will NOT be available on PM Pediatric Care Telemedicine

How do I set up a portal login for a patient?

You will need your phone number and the patient’s date of birth to complete the setup and it must match the information provided for the patient’s chart. The email subject line will be “Portal login information from your doctor’s office.” If you don’t see the email, check your spam/junk folder.

How can I access my child's health information 24/7?

Your child's health information and your account information are available 24/7. Send an encrypted non-urgent message to your healthcare provider or staff member Pay bills (by phone, if you’d like). Even pre-pay copays.

Sports Physical Forms

If your child is going to need a sports physical form completed during your appointment with us, please notify the front desk when you check-in.

All Other Forms

If you have a form you need completed for your child, please bring it to the office with any pertinent instructions. Please allow 5-7 days for form completion and all forms are subject to a $25 fee.
