The Phoebe Patient Portal is a convenient way to manage your personal healthcare and connect with your doctor. This online tool securely stores your medical history, medications, personal information used by doctors and nurses along with inpatient records, lab results and more. Access your records at any time.
If you experience trouble logging in or accessing Phoebe Patient, please call 229-312-5465, Monday – Friday from 8 a.m. – 5 p.m. If you register for Phoebe Patient and forget your password, you can simply click the “Forgot Password?” link below the sign-in box on the sign-in page.
You can also email HIMROI@phoebehealth.com to request your medical records. Please include a call back telephone number, patient’s full name, date of birth, the visit date of the records needed, and the address where the medical records can be mailed or if pick-up is preferred.
Therefore, we are excited to announce a renovation of our main entrance at Phoebe Putney Memorial Hospital in Albany. While our lobby is under construction, the main entrance will be relocated (see map). There will be signage directing patients and visitors to this temporary entrance. Phoebe Putney Health System will make available public records in compliance with Georgia’s Open Records Act. Acknowledgement of the request will be made within three business days and either make requested records available for inspection, provide copies, or give a reasonable estimate of when the requested information may be available.
All requests for public records must be submitted in writing, including facsimile transmission or email, to the Open Records Officer. The request should include name of the requester, contact information and information necessary to readily identify the public records requested.
