Selection of multiple forms ensures that the patient will get all of the necessary patient forms to fill out for the given scheduled visit. You can manage the entire suite of forms assigned and sent to the patient. You can also remove or retract a form that has been sent, but is no longer required.
Patients can securely read, edit, and sign personal information and documents that are automatically added to their chart. Patients can sign into any mobile device that has the AdvancedMD patient kiosk app installed, even the same device that the admin kiosk app is installed on.
Portal forms can be accessed from the homepage of the portal. The Patient Forms icon lets you see which family members have outstanding patient forms assigned.
The portal user can determine which patient or family member has the patient form completed. Click the line item to access the form.
You can preview forms that have been submitted by the patient/responsible party to accept, reject, or remove. Accepting the intake and consent forms brings the content of the form into AdvancedMD EHR as a chart document. Rejecting the intake or consent form will send the form back to the patient in either the Kiosk or Portal applications.
