ADPH is a public health authority as defined by the Health Insurance Portability and Accountability Act (HIPAA), Standards for Privacy of Individually Identifiable Health Information ; Final Rule (Privacy Rule) [45 CFR §164.501] to collect or receive protected health information (PHI) for the purpose of surveillance, investigations, and interventions of notifiable diseases, without authorization of the patient. Immediate, Urgent Diseases/Conditions must be reported to ADPH within 24 hours of presumptive diagnosis either electronically, telephone (800-338-8374), or fax. Standard Notification Diseases/Conditions must be reported to ADPH within 5 days of diagnosis (unless otherwise specified in the Rules) electronically or in writing.
REPORT Notifiable Disease is a reminder for all reporting entities to report notifiable diseases in a timely manner. REPORT is vital to ensure Alabama's Notifiable Diseases are investigated quickly to reduce further possible exposure and to treat those infected.

Electronic Health Record. The Electronic Health Record (EHR) is a computer-based clinic management system. EHR created and maintains an electronic version of a clinic patient’s paper chart in real-time, and it is accessible statewide.
EHR is compatible with scanners to scan in drivers licenses for the banner on the screen and insurance cards, signature pads for patients’ use, and bar code scanners for inventory control! All of this allows us to: Streamline the appointment process and clerical procedures.

New patients are asked to arrive 30 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results, photo ID and your insurance card to your appointment.
Please contact your pharmacy for refill requests. Your pharmacy will fax us the request and your refills will be processed within 72 hours. You may also request refills on the patient portal.
We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards. (Visa, Master Card, American Express, Discover.)
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.
To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.
