To get to your patient portal, you’ll need to contact your medical provider’s office for the web address. They can also provide you with instructions on how to register and log in if you need them. Please Note: All YourTown Health clinics offer similar services and do not require an appointment to be seen however, those with an appointment will be seen before walk-ins. If you don’t have health insurance, you may be eligible for the Sliding Fee Scale. This scale discounts your bill based on your household income and number of people in your household.

We are opening our own in-house pharmacy that can take advantage of special pricing available only to Community Medical Centers. We can then pass these savings on to you. Also, we have a full-time nurse who manages our Patient Assistance Program. If you have any concerns about affording your medications, please talk to your provider or nurse.

If you experience difficulties trying to log in to MyChart, please close all browser windows, open a new browser and try again.

Who can I message in MyChart?from yalehealth.yale.edu

When you activate your MyChart account, you will immediately be able to send secure messages to your primary care clinician in Internal Medicine, Student Health, and Obstetrics & Gynecology. If you have proxy access for your child, you will be able to message their primary care clinician in Pediatrics. Please note proxy access for children is not available for children aged 13-17.

When can I see my test results in MyChart?from yalehealth.yale.edu

Results of common lab tests are immediately available in MyChart. Other tests are released 3 or 7 days after finalized , including imaging tests and pathology. Many providers will manually release them earlier, often with explanatory comments.

Are hospital admissions available in MyChart?from yalehealth.yale.edu

If you are admitted to a hospital in the Yale New Haven Health system, you will be able to view your schedule of medications and testing that take will take place throughout your admission. Once you are discharged, you will be able to view your date of admission and discharge along with your discharge instructions.
