York General believes that every patient should have easy, instant access to his or her health information at any time. We are pleased to present this opportunity through the use of a secure internet portal.
To request your medical records please print off form, (button below) fill it out and sign. Bring it with you to the HIM department or fax 402-362-0452. Please be sure to include yourself and anyone else you give permission to pick up your medical records.
Have someone in your life who helps manage your healthcare? York General needs patient consent for others to have access to their medical records. For example, if a daughter wants to look at her adult parent’s information, York General would need written consent from that adult parent.
Please note that you can only review your health data, the portal is not used for two-way communication with York General or your provider.
In celebration of Doctors' Day, we want to highlight our medical director of Encompass Rehabilitation Hospital of York, Dr. Hejab Imteyaz! Dr. Imteyaz is a physiatrist. A physiatrist is a doctor who specializes in physical medicine and rehabilitation, using a comprehensive approach in treating the entire patient, not just symptoms. Dr. Imteyaz’s favorite part of her role is the team approach taken in treating our patients, and of course, seeing the positive changes in the patients that enable them to meet their goals in rehabilitation. Dr. I received her resident training in Physical Medicine and Rehabilitation at Albert Einstein College of Medicine in New York. We are very fortunate to have such a highly trained and devoted physician as our medical director overseeing our patients’ care. Happy Doctor’s Day DrI!

Patients can join any day of the week during their stay. Each day of the week has a specific topic which incorporates the AADE 7.
