Y Access Transfer Center offers a streamlined 24/7 patient transfer service that ensures quick and easy admission to a Yale New Haven Health hospital. With one call to 888-YNHH-BED (888-964-4233), referring physicians can receive attending physician consultation or assistance from trained paramedics for a patient transfer.
Within two hours of admission to a Yale New Haven Health hospital, you can log on to EpicCare Link to access a patient’s medical record. This includes previous hospital admissions, clinic or Emergency Department visits at a Yale New Haven Health hospital.

In order to receive Epic access, a user must first be assigned an Epic ID and go through Epic training. To request an Epic ID, contact the Yale New Haven Health Help Desk and request an Access Service Request Form. Processing the Access Service Request form may take up to 10-15 business days.

If you are a patient and do not want this information released, please call the Admitting Office at 203-688-3331 or ask the patient service manager on your unit to make those arrangements.
