Everyone must have an appointmentbefore entering the Yale Health Center (except for Pharmacy and Acute Care visits). Visitors are not allowed at this time. One parent may accompany their child to their appointment. Everyone entering the Yale Health Center is required to wear a mask, even if you are vaccinated.

Sign up for a MyChart account: Or you can ask the receptionist at your next visit to email you a code or go to the Member Services desk and ask to be emailed a code. Or after your visit you will receive an After Visit Summary. The last item on the sheet is the MyChart information with your activation code.

Call 1-855-513-5513 and a MyChart representative can provide a new activation code. During your first session, you'll be prompted to activate with your code, create a MyChart ID, a new password and choose and answer a security question.
To get the app, you need an iPhone®, iPadtm, or iPod touch®. From one of those devices, go to the App Store application and install "Epic MyChart". From and Android device, go to the Market application and search for "Epic MyChart".
In order to receive Epic access, a user must first be assigned an Epic ID and go through Epic training. To request an Epic ID, contact the Yale New Haven Health Help Desk and request an Access Service Request Form. Processing the Access Service Request form may take up to 10-15 business days.
You may submit the form via email at yhmedicalrecords@yale.edu, by mail to Yale Health, P.O. Box 208237, New Haven, CT 06520-82327 or by fax to 203-436-5536. The cost for a copy of your medical record is a flat fee of $6.50.
If you have an e-mail address on file then your MyChart Username will be sent to your e-mail account. If you do not remember any of this information, or you do not have a valid e-mail address on file, you will have to contact your MyChart help desk at 866-385-7060 to help you regain access to your MyChart account.
Request an Activation Code We need some information to grant you a MyChart account. After you submit this request, it may take up to a week for processing and verification. When you're approved, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account.
