If you previously established an account for online access to your health record and need to gain access to that account again, you will not qualify for self-enrollment. Click on the Patient Portal Frequently Asked Question (FAQ) link in the sidebar on the right for more information on recovering access to your health record.

Patients of WakeMed Physician Practices can use WakeMed MyChart to: 1 Schedule or request appointments with WakeMed Physician Practices providers. 2 Save time by filling out health history information before your appointment. 3 View details from all outpatient appointments, including lab results, testing, diagnoses, medications, etc. 4 Communicate with your outpatient providers and request prescription refills by the click of a button.
To request proxy access, you will need to either log in to or create your own . MyChart account. After your account has been established, you can find the Request Proxy Access Form under the "My Medical Record" tab. For minors, proxy access will terminate when the patient turns 13 years old.
