How do I submit my trip log to MTM?

Reimbursement Trip Log Instructions: • You must call MTM at 877-633-8747 at least two business days (within MTO region) or five business days (outside of MTO region) before the day of your medical appointment. Mileage Reimbursement Logs

Use a trip log to track your travel. Instructions are on the log. Have your medical provider sign the log for each appointment. There is no need to track mileage. Our system will do it for you.
